About the Role

Newport Pagnell Medical Centre are looking for a Senior Nurse for their Community Health and Support Team.
This position is offered on a part-time basis working 21 hours per week.
 
This is an exciting role that would see you working alongside a forward thinking and supportive practice-employed integrated nursing team and dedicated administration team.
 
Duties would include the following;
  • Provide clinical leadership and operational oversight for the Integrated Community Support Team (ICST) and Community Matron functions
  • Proactive, co-ordinated and holistic care for patients with complex health and social needs
  • Aim to improve health and wellbeing, preventing unnecessary hospital admissions, reduce frequent GP attendances
  • Supporting patients to remain living well at home
  • Drive integration between ICST and Community Matron services
  • Line management of the team consisting of a nurse, HCA and a Care Co-ordinator - conducting annual appraisals, mentorship and professional development
  • Service development
  • Collaboration - working closely with GPs, social care, frailty teams, district nurses and voluntary agencies
  • Represent the service at MDT and external forums

About the Candidate

All applicants should have the following qualifications, registrations, skills and experience:
  • A relevant nursing degree.
  • Current NMC registration with an active PIN.
  • Experience of supervisingstaff/team management is preferred.
  • Experience of working across health and social care boundaries.
  • Compassionate manner with an empathetic approach and demonstrable kindness in all aspects of your work. The successful candidate will value the dignity of our patients, their relatives and carers, as well as their colleagues at the practice.
  • Experience in community nursing and managing long term conditions.
  • Insight and understanding of current issues in nursing and the wider NHS.
  • Evidence of service improvement or audit involvement.
  • Evidence of continuing professional development
  • Advanced clinical assessment skills

We are a large training practice based in the town of Newport Pagnell with another surgery in the Willen area.
