Advanced Nurse Practitioner

East Hill Surgery located in Colchester, are seeking an experienced advanced nurse practitioner to join our friendly and supportive team. We are ideally looking to recruit on a full-time basis but are happy to consider 30-37.5 hours per week.

The role will offer you the opportunity to build on your experience for working effectively within our multi-disciplinary healthcare team as you will work alongside fellow professionals within the practice to support one another.

The main responsibilities of the role include:

• Respond to referrals and provide assessment, diagnosis and treatment at first point of contact by attending patients either in the practice or patient home.
• Support and continue the development of our on the day emergency patient service in the practice.
• Triage and prioritise home visit requests.
• Evaluate clinical information from examination and history taking, and initiate appropriate treatment and/or referral.
• Maintain accurate clinical records in an efficient manner.

• Advanced nurse practitioner, with experience ideally in primary care (but not essential).
• NMC registered nurse.
• Ability to effectively assess, plan, treat and refer patients, and evaluate patient care confidently.
• Experience of working to protocols/guidelines and patient groups/patient specific directives.
• Excellent communication skills.
• Experience of autonomous practice working and multi-professional team working.
