About the Role

The Deben Health Group PCN is looking for an enthusiastic, organised and compassionate Care Coordinator to join our award-winning, friendly multidisciplinary team. This position is offered on a full or part-time basis working up to 37.5 hours per week.

Supporting approximately 38,500 patients across Wickham Market Medical Centre, The Peninsula Practice, Little St John’s Street Surgery and Framfield House Surgery, you will help people navigate services and access timely, coordinated support.

This varied role combines administrative care coordination with social prescribing support.

You will carry out the following duties:

Liaise with patients, carers, practice teams, community services, care homes and voluntary organisations
Coordinate referrals and follow-up actions
Support multidisciplinary meetings
Connect people with services that can improve their health and wellbeing

About the Candidate

If you are a skilled care co-ordinator, with excellent person-centred planning experience and strong local knowledge for accessing key support, then we would love to hear from you.

The ideal applicant will have the following skills, experience and qualifications:

Has excellent administrative, organisational and communication skills.
Enjoys helping people and can provide a calm, respectful and non-judgmental service.
Can manage competing priorities, maintain accurate records and follow actions through.
Works confidently both independently and as part of a multidisciplinary team.
Understand confidentiality, safeguarding and the importance of professional boundaries.
Has experience in administration, customer service, healthcare, social care, community services or a similar role.
Can travel between our member practices and community locations.

Deben Primary Care Network support approximately 38,500 patients across Wickham Market Medical Centre, Peninsula Practice, Little St John Street Surgery and Framfield House Surgery.
